This Privacy Policy explains how we collect, use, share, and protect information when you visit our website, complete our qualifier form, contact us, or use our services.

GetfitRx is a wellness and coaching company. Medical services such as evaluation, prescribing, and ongoing medication management (including GLP-1 therapies) are provided by licensed independent medical providers. Information you submit may be shared with those providers in order to evaluate your eligibility and deliver care.

2. Information we collect

We collect the following categories of information:

• Contact & identity information you provide through our qualifier or contact forms, such as your first name, email address, and U.S. State of residence.
• Health-related information you choose to share, such as whether you are currently using a GLP-1 or peptide, whether you are interested in starting one, and your interest in our community or in-person services. You are not required to share clinical details through this site, and we ask that you do not submit detailed medical history through public forms.
• Routing & eligibility data derived from your responses, including whether you appear to qualify for local in-person services based on the state you select.
• Technical information such as IP address, browser type, device information, pages visited, and referring URL, collected automatically through cookies and similar technologies.
• Communications you send us (email, phone, or form submissions) and our responses.

3. A note about HIPAA and PHI

The qualifier form on this website is a marketing and intake tool, not a medical record. Information collected here is used to determine whether GetfitRx and our partner medical providers may be a fit for you. Once you become a patient of one of our partner providers, that provider's notice of privacy practices under the Health Insurance Portability and Accountability Act ("HIPAA") will govern protected health information ("PHI") created and maintained as part of your care. Please do not submit detailed clinical information through this website.

• To respond to your inquiry and route you to the appropriate program (community membership, virtual consult, or local in-person care).
• To determine, based on your state, whether you may be eligible for in-person services at our Cranston, RI office.
